I had this happen when i changed from a wired to wireless head set what i did was go to control panel > sound. then there was a list of things and it said my wireless was unplugged when it wasn't so i disable my wired (just disable what ever is enabled) then once you do that the volume icon at the bottom right of your screen will get a red ex on it once it does that click on it and then it will say troubleshooting. click on that and let it troubleshoot and then it should automatically enable what ever you have plugged in.

You don't know how annoying it is as a T to be ordered to crouch all the way somewhere and your hand slips off of the crouch button and you press it again and then get and awp to the face. Now how did the T uncrouching for a millisecond endanger the CT? That CT was just looking for an easy kill. Really its only situation like these that should require warning shots. If someone is running the opposite way that you told them or going for a secret or vent then fine shoot them but for stupid rules which are created to get easy kills, warning shots should be required.
