That is just how our server is.
We have a lot of rules in place, that restrict what we can and can't do to thin out the terrorists, and it is rare for a CT to have the balls to keep moving everyone around the map, giving them chances to rebel in a semi-effective way, where now basically we cant rebel until some CT is dumb enough to stand in the doorway, or we are dumb enough to go into vents/tele's/out which are always being camped hardcore by pussy's.
One of the major differences is in most servers, death games are used to thin the T team down to just 2 LR's it is kind of sad, but that is just how it is. I remember all my friends playing in 187ci's PB and they dragged me there once, it was always the same map, always the same orders, always the same result. Cells open, report to the cage in the center of the room, get in the pit, get out, get along the fence, un-stack, evil simon says/simon says/last reaction/first reaction, whole team is murdered as fast as the ct's can do it, last 2 get their lr's. This was a server with a 9 minute round time, and most round where done in 3. It sucked.
I personally hate Death Games, I just don't find them fun, more just thinning the herd, but it is something that can be used in a good way. But it does build a pretty different PB, And we do have a lot of players who like that style of PB.
You can still put a weapon restrict in the maps, which would restrict awps.
