T's talking over ct's

Simples answer: Give orders regardless of if a T is talking over you, if and WHEN they don't follow them...shoot them...when they ask why say rules were given, if they say they didn't hear you...tell them to stop talking over you...

they should make deathgames alright even if T's don't want to play them, it moves the round along and they're fun

Deathgames are fun for the cts, not the ts. If they were fun for the ts then they would agree to them a lot more.

sm_mute @t for 20 seconds after the round begins, then unmute them. A script on the server could do this. 20 seconds is enough for orders, and everyone can hear them.
