Yes, it appears to be a Source-Engine bug. I have reported it to Valve and hopefully they'll roll out a patch soon. In the meantime, spam it a few times (once every few seconds) and it will work eventually. It appears to be heavily rate limited.

No. CTs need to stop being trolls and decide which order they will follow. This works 99% of the time, unless (not saying names here) joins the server and intentionally gives orders other than what the other CTs agreed on, just to cause trouble. The server is as near to PERFECT as it will ever be, right now. Please don't mess with it anymore . You've done a wonderful job.

Is there an option for "fukken awesome"? It's a LOT more balanced now. I still don't shoot them unless they are obviously rebelling (ie: go to pool, crouch, hug right wall, no left side; if they stand up for a second, fine, they probably sneezed or something. If they go running off, then I shoot them). It's wonderful new rules. Fun for everyone is not "Ts win every round". Now CTs have a chance, and teams are a lot more balanced. I hope the new rules stay on forever 100% agree with Desum, even though I am a "newfag" who never played 08 PM, I like fun but I am still a strict bitch.

Guize this is seriows and important: You need to run a Mumble server instead of Ventrilo... you're never going to be able to fit everyone who will probably be there into the current Vent. Mumble has no slot limits, and it's a free open-sourced program that is similar to Vent, only without the proprietary restrictions.
